But if you have LVM , at step 3 you need : 

#vgscan 
( system vg should be discovered) 
#vgchange -a y system
#mount /dev/system/root /mnt
#vi /mnt/etc/shadow

Also step 10 you need to mount /usr before to run passwd command 
#mount /usr
